The purpose of this job:
Bupa Telehealth provides health services designed to support our customers health. These services include those to help customers manage chronic conditions and those to provide support around a hospital admission.
These services are delivered by our Health Coaches. A Health Coach delivers a person-centred behaviour change program designed to help our customers better manage their health and navigate the health system.
The Health Coach provides a range of current, evidence-based, best practice health information and support to customers. This support is primarily provided over the phone.
The primary focus is to motivate and inform a customer regarding their health and wellbeing choices in order to become a more active and engaged participant in the management of their own health.
What you’ll need to make it possible
- Tertiary qualifications in a relevant health field including, but not limited to, Dietetics, Physiotherapy, Exercise Physiology or Psychology (including current, unrestricted AHPRA registration, accreditation with Dietitians Australia or equivalent).
- An understanding of clinical best practice in chronic disease management
- Experience working within diverse clinical acute and/or community health setting where sound clinical judgment essential
- Strong communication skills with a demonstrated ability to educate and motivate individuals
- Proficient in windows-based computer software and ability to work simultaneously and efficiently with telephone-based duties
- Strong customer service skills, commitment, and a desire to satisfy customer needs
What you’ll be doing in this role:
- Use person-centred behaviour change techniques and other coaching tools, motivate and inform customers in how to better manage their health
- Assist customers to develop action plans to address opportunities for health improvement.
- Guide customers to build on their knowledge, skills and confidence to support self-management of their health.
- Manage individual caseload of customers scheduling and achieve productivity targets
- Deliver the relevant service in accordance with the Bupa Telehealth Quality Framework and program guidelines and standards.
- Maintain clinical documentation in line with professional legal and regulatory requirements and Bupa Telehealth standards.
